Patient Resources ~

We require both the New Patient Form, and the Patient Release Form to be completely filled out in order to establish you as one of Dr. Ritz’s new patients, and to schedule an appointment.  This information becomes part of your patient chart.

To facilitate your visit, we offer convenient ways for each patient to provide private health data to our office.  We suggest filling our the New Patient Form and Patient Release Form here through our site, which then allows you to email them directly to the office for expedited processing.  Should this option not suit you, you can print out forms and fill them out at your leisure, then either fax them to our office at (970) 259-5934 prior to your appointment, or bring them with you when you come in for your appointment.

Kindly note our strict Cancelation Policy: If you cancel less than 24 hours in advance of your appointment, you are charged $70 for the time lost.  This is due to the fact that our schedule is booked far in advance, and appointments not kept create empty space of lost time in Dr. Ritz’s schedule.
